Concert Rental Form
We would like to make your concert booking as smooth as possible. Please take a moment to read the details below and fill out the form as thoroughly as possible. Starting 2024 the following Service Charges will apply to all concert bookings: Custodian = $50/hour with a minimum of a 4 hour charge. Majority of concerts need approximately 10 hours. Technical, Sound and Lighting = $75/hour with a minimum of a 4 hour charge. Majority of concerts need 10 hours. Food Handler = $50/hour with a minimum of a 4 hour charge. This fee will only be applied if food is served AND if you can not supply your own Food Handler and their certificate. Beginning in 2024 a charge for each room booked will be applied per day. Main Auditorium = $750.00 (non ticketed) Main Auditorium = $2,000 (ticketed) Main Lobby = $0.00 Restrooms = $0.00 Green Room = $0.00 Each additional Room =$100.00 Parking Lot = $0.00 Forward Church will provide the following services at no charge: 1. Laundry facilities, including detergent 2. Overnight parking 3. Unlimited WIFI usage - for organization renting Forward Church and their staff only 4. Unlimited access to change rooms, Green Room and private restrooms. 5. Carts and trollies of different sizes to aid with unloading and loading of equipment 6. Tables and coverings for merchandise and ticket sales 7. Access to the code for our coded doors in the staff hallway and Green Room (this is not be shared with any people beyond renters staff and volunteers) Forward Church will NOT provide volunteers, meals or snacks. Forward Church has the right to be the sole provider of a concession stand providing snacks and beverages. A Certificate of Liability Insurance (minimum of two million dollars) must be provided to Forward Church within 10 business days of receiving the Tentative Booking email from Forward Church.
