Graduation and Commencements Form
We would like to make your booking experience as smooth as possible. Please take a moment to read the details below and fill out the form as thoroughly as possible. The following Service Charges will apply: Custodian = $50/hour with a minimum of a 4 hour charge. On average commencements need approximately 10 hours. Technical, Sound and Lighting = $75/hour with a minimum of a 4 hour charge. On average commencements need 10 hours. Food Handler = $50/hour with a minimum of a 4 hour charge. This fee will only be applied if food is served AND if you can not supply your own Food Handler and their certificate. A charge for each room booked will be applied. We are offering the rooms at a discounted rate for all graduations and commencements: Main Auditorium = $750.00 Main Lobby = $0.00 (does not include the cafe) Restrooms = $0.00 Gymnasium = $150.00 Cameron Hall = $300.00 Kitchen = $100.00 Parking Lot = $0.00 Each additional Room =$100.00
Please note: Forward Church has many ministries running throughout the week. If your booked rooms are available, you will have full access to them starting at 9:00 am for setting up excluding the gymnasium. Access to the gymnasium will open at 3:00 pm. Due to the volume of graduations and commencements, we can only accommodate your rehearsal on the same day as your event.
A Certificate of Liability Insurance (minimum of two million dollars) MUST be provided to Forward Church within 10 DAYS of receiving your Tentative Booking email from Forward Church.
The Church reserves the right to cancel any rental agreement that does not comply with the terms and conditions in this policy or is in conflict with the bylaws, policies, or general purpose and beliefs of the Church.
Any rental must not have activities contrary to the general purpose and beliefs of Forward Church. For more details, please see our Articles of Faith and Lifestyle Statement.

The food premise has not been inspected in accordance with the Ontario Regulation 493/17 Health Protection and Promotion Act. It is the Renters obligation to keep a list of all persons who donate potentially hazardous food for this event and the renter must provide a copy of that list to a public health inspector on request. The list must contain each donor's name, address and telephone number, in full.
The renter will not allow any damage to be committed on any portion of the property or building. The renter will need to return the premises in good condition, as it existed at the start of their event, ordinary wear and tear excepted. The cost and expense of repairs necessary to restore the condition of the Premises will be covered by the renter.
